Acne are caused by bad hygiene

Why does advertising work?

Here’s a number to start us off. Marketers spend about $70 billion a year on TV ads! That’s a lot of cash! But companies out to make a profit would not spend that much on ads unless there was good evidence they work. Except, what is it, exactly, that ads do? The myth is marketers want you to buy a product the first chance you have. It’s like the old movie joke about cinemas so desperate to boost sales of ice cream, they would turn up the heating during the first feature and trigger a rush for something cool by showing an ad at the start of the intermission. The majority of ads are designed to sit in your memory. It’s like the Geico ads with the caveman or, more specifically the pig who cries wee all the way home.

There’s good evidence that hard selling techniques create a negative impression. We all take some pride in our ability to resist. So the aim is to build up brand awareness. Over time, we associate each brand with the product or service. The challenge is then to convert these memories of an amusing ad or catchy slogan into action. After all, why go to all the trouble of planting the seed if you don’t then go on to encourage people to buy later on? Except, marketers have no real control over our actions. Most of us use the ad break to make a quick cup of coffee or empty the bladder of all the coffee we’ve drunk over the evening. No one ever sits down to watch the ads. All we do is catch glimpses of the ads in passing. This constant repetition is like drip-feed, slowly building up brand awareness so that, if we’re making a decision on which product or service to buy at some time in the future, we might buy the one with the most memorable ad.

Creativity is what sells brand awareness. We remember the funny ads, or the strange ads, or the visually interesting ads, or the ads with the best music tracks. So now imagine you’re going to be paid a big chunk of cash to create a memorable ad for an erectile dysfunction drug. If you’re selling a car, you can show it accelerating from 0 to 60 in five seconds. Imagine the response of the television stations if you showed the pill producing an erection in record time. That would really test the First Amendment. So you need to sneak up on people and, hopefully, slip past their defenses before they know you’re there. (more…)

Is change possible?

The latest report from the Census Bureau has produced some worrying figures. It seems almost 17% of us are living below the federal poverty line. You have to go back to 1993 to find a higher percentage of Americans living in poverty. To add to the interest, the Super Committee in Washington has just begun its discussions on what to do about the federal deficit. Its choices are between increasing revenue to maintain more of our services or simply cutting spending. Given the terrible state of our economy, it’s hard to see what the federal government can do to help the poor. In this, there’s a secondary consequence looming into view. No matter what happens to Obamacare in its journey to implementation in 2014, there are an increasing number unable to afford health insurance. It’s currently estimated some 52.5 million adults are without any form of support from Medicare, Medicaid or VA benefits. This reflects the dramatic and sustained increases in the costs of treatment and care. We now have the highest retail prices for drugs, medical devices and hospital treatment in the world.

The European approach to pain management is evidence-based. In other words, the regulators have invested time and money to research which treatments are the best value-for-money. These treatments are then made available either free or at subsidized rates. All other treatments are left available but the patient must pay the full price, whether through private insurance or out of savings. The evidence consistently shows counseling in general and cognitive behavioral therapy in particular, are the best long-term strategies for pain management. In labor terms, this one-to-one approach is relatively more expensive than drug therapy in the short-term. But, if you look at the continuing costs of drugs and the added costs of dealing with problems of dependence and addiction, paying more at the initial stages of treatment gives a better outcome.

Unfortunately, this is not going to be rolled out in the US. First, it’s against the interests of the drug manufacturers. At the current high retail prices, their losses would be spectacular if doctors and therapists referred their patients to cognitive behaviour therapists. So the lobbying to doctors is firmly in place to keep the flow of prescriptions running smoothly. Second, the doctors would lose a share of the revenue to therapists. As things stand, it’s easy money to see multiple clients an hour and write prescriptions. Unless there’s a culture change, doctors are unlikely to cooperate. The only hope for forcing change comes from the health insurance industry. As more people are unable to pay their premiums, the profitability of insurers is being squeezed as health costs rise. They may decide to prefer paying for preventative medicine and cognitive behaviour therapy as the pain management strategy. Such is the insurers’ buying power, they may force some change. There are other problems

In the bad old days before the little blue pills burst on to the market, no one liked to talk about sexual problems. It was all just too embarrassing, particularly because we all knew there was little medical science could do to solve the problems. So all this embarrassment was put into a box with the label “impotence” stuck on to the outside and it was put away on a high shelf where no one could see it by accident. Except, of course, this didn’t stop men from finding they did have problems and, if the news leaked out, these men found themselves the butt of jokes and, worse, mockery. We’ve never been very kind to each other when we all fear the same problems. So, working quietly in labs around the world, teams of researchers have been trying to solve a range of different sexual problems. For example, there are men who can achieve a hard erection but cannot then reach an orgasm. This is deeply frustrating for the man and, if the woman is interested in pregnancy, somewhat annoying. Less obvious but no less annoying when it becomes apparent are those men who do have an orgasm but do not ejaculate, or only produce very low volumes of sperm, or do not produce active sperm. Fertility is something that can only be proved by scientific test and it’s not unknown for women to ask for sperm samples for testing before agreeing to marriage. All this before we get to the men who don’t produce an erection.

Sex is a complicated area of medicine because it mixes physical ability into social situations. If the body fails, it may be because the mind is not involved. It’s a fact no man produces an erection unless he finds the situation sexually stimulating. So if he’s not genuinely attracted to the woman, there will be no erection. That’s why erectile dysfunction and depression are always linked. When the man grows too sad, he fails to find any situation sexually simulating. When this happens, the only medical solution is treating the depression. None of the erectile dysfunction drugs will have any effect until the mind heals itself.

Unfortunately, there are no signs of any breakthroughs on the horizon on any of these other problems. That’s why, with more men experiencing low fertility, there’s increasing pressure on adoption agencies. If there are not enough children locally, many Americans have been adopting children from abroad. This has become controversial as have agreements for surrogates to bear a child for a couple. Yet, without there being any prospects of a quick medical fix, childless couples are finding life increasingly difficult as the number of children born is also declining. Even though the pro-life movement has reduced the number of abortions, this has had no real effect as more couples have been deciding either not to produce children or delaying it until later. (more…)
